Abstract

Objective: Implementing an alternative education method on the perianesthesia units at a Bay Area Hospital to improve nurses’ confidence on the topic of malignant hyperthermia (MH). Reviewed literature has shown the positive outcomes of escape room simulations on nursing education. Aim: This quality improvement project aims to increase the confidence of experienced registered nurses in recognizing and responding to malignant hyperthermia by achieving a 25% increase in the number of nurses reporting improved confidence scores, measured through pre- and post-intervention surveys, after participating in the malignant hyperthermia escape room education program in the perianesthesia units at a hospital in the Bay Area in California by August 10, 2026. Methods: Perianesthesia nurses will complete an escape room-based simulation at a Bay Area hospital. Confidence scores will be measured via pre- and post-intervention surveys. The specific areas of improvement include the topic of MH, caring for a pediatric patient experiencing MH, recognizing the early signs and symptoms, locating and preparing the MH emergency cart, and preparing and administering dantrolene during an emergency. Results: 88.9% (48 of 54 nurses) of nurses showed an increase in confidence after participation in the MH escape-room educational simulation. 9.3% of nurses showed no change in confidence, and 1.9% of nurses showed a decrease in confidence after the simulation. Confidence improved in all five survey areas, and the largest improvement was seen in nurses’ confidence regarding preparing and administering dantrolene for MH events. Conclusions: The project's aim of 25% of nurses whose confidence scores increased after the simulation was met and exceeded. Overall, the escape-room simulation increased nurses’ confidence in all specific areas surveyed. From May 2026 to August 2026, there have been no MH events in the perianesthesia units.
