Abstract

Objective Implementing an innovative technique at a Bay Area Hospital in the perianesthesia units to improve nursing education on the topic of malignant hyperthermia. Available literature has shown the positive effects of implementing an escape room-based simulation in increasing clinical competency, critical thinking and problem-solving skills, and preparedness for various clinical scenarios. Additionally, learning with the escape room approach has shown increased satisfaction and enjoyment with the learning experience. Aim The quality improvement project aims to increase experienced pediatric registered nurses’ confidence in the topic of malignant hyperthermia by achieving a 25% increase in the number of nurses reporting improved confidence scores by August 10, 2026. Methods At a Bay Area hospital, perioperative pediatric nurses will complete an escape room-based simulation. Confidence scores will be measured via pre- and post-intervention surveys. Specific areas of improvement include the topic of malignant hyperthermia, care for a pediatric patient experiencing malignant hyperthermia, recognition of the early signs and symptoms, locating and preparing the malignant hyperthermia emergency cart, and preparing and administering dantrolene during an emergency. Results 88.9%, or 48 out of 54 nurses, showed an increase in confidence after participation in the escape room. Confidence improved in all five survey areas with the largest improvement in confidence regarding the preparation and administration of dantrolene for malignant hyperthermia events. Conclusions The quality improvement’s goal of a 25% increase in confidence scores was met and exceeded. From May 2026 to August 2026, there were no malignant hyperthermia events in the perianesthesia units.
