Abstract
Objective: Intimate partner violence is a widespread public health concern, with an estimated 64.2 million U.S. adults affected in their lifetime and significant physical, psychological, and economic consequences. Due to systemic challenges, IPV screening remained inconsistent across an adult primary care clinic serving a predominantly monolingual Spanish-speaking population in the San Francisco Bay Area, with only 4.1% of patients screened across the Adult, Women's, and Teen clinics in June 2026. Aim: This quality improvement project aimed to increase overall IPV screening by 10%, from 4.1% to 14.1%, by July 31, 2026. Methods: Standardization efforts included introducing a printed Hurt, Insult, Threaten, Scream tool in English and Spanish in conjunction with ancillary interventions. Interventions included a 60-minute in-service training for Medical Assistants and Registered Nurses, IPV scripts for patient disclosure and behavioral health referral, and efforts to expand electronic health record Care Gap changes to include IPV screening for male-identifying patients. Results: Pre-intervention data collected in June 2026 was compared with post-intervention data from July 2026. Following implementation, IPV screening rates increased from 4.10% (103 of 2,513 patients) to 7.20% (199 of 2,774 patients), a 93% relative increase in the number of patients screened. Conclusion: Although this threshold did not meet the project's estimated 10% increase, this improvement indicates a positive foundation for growth in the microsystem level. These findings suggest that a multi-intervention approach combining standardized screening tools, staff education, and structured scripts can meaningfully improve IPV identification in a culturally and linguistically diverse primary care setting.
