Abstract

As Florence Nightingale once said, "The very first requirement in a hospital is that it should do the sick no harm." This principle is at the foundation of nurse-led clinics, which provide safe and effective care to those who need it the most. This literature review explores how nurse-led clinics are transforming healthcare delivery and embodying Nightingale's commitment to patient safety and well-being. These clinics have demonstrated a remarkable ability to reduce hospital readmissions by up to 30%, highlighting their effectiveness in managing patient care. The primary objective of this project is to enhance patient care through nurse-led interventions for vulnerable populations, including the elderly and veterans. By focusing on primary and secondary prevention, these clinics offer essential nursing services such as screenings, assessments, and monitoring of vital signs, which are critical for the early detection of health issues. Early identification allows for timely interventions, significantly reducing the risk of complications and ensuring that underserved communities receive the appropriate healthcare services they need. This improvement is vital for preventing minor health issues from escalating into severe conditions, ultimately enhancing overall health outcomes.
