Abstract
Descriptive Abstract
Purpose: To improve patient care and help reduce CHF readmissions by addressing CHF patient education among nursing staff.
Specific Aim: We aim to improve patient self-care and safety, and decrease the readmission rate for persons admitted with CHF at MSJMC by enhancing the process and content of current CHF patient education. Nursing staff will report a 100% confidence rate and intent to provide needed CHF education, and a long-term CHF readmission rate goal of 22% will be achieved by December 31st, 2016.
Methods: Five proposed activities were to: 1. Increase nursing knowledge of patient resources; 2. Advertize free community CHF class on May 5th; 3. Create ‘Heart Failure Zones’ patient education standardization; 4. Observe staff nurse consistency with the teach-back method; and 5. Assess staff nurse perception of an evidence-based checklist trial.
