This project focused on the Clinical Nurse Leader (CNL) essential of Quality Improvement and Safety. I used the CNL competency of using performance measures to assess and improve the delivery of evidence-based practices for this project. The primary care practice in which I work received data that revealed almost 44% of the diabetic patients have a Hemoglobin A1c (HbA1c) >8%. I aim to assist these patients to reduce their HbA1c to
